IAP offers you a variety of ways to apply for low-cost group insurance coverage...


Apply Online

To complete an online application for Term Life, Dependent Children Term Life, Accidental Death & Dismemberment and/or Critical Illness Insurance, follow these three easy steps:


1.


Open the Online Application page.


2.


Complete the application in full.


3.


Choose to either submit your application online or print out a copy and mail it to IAP.

To use IAP's online application, you must have JavaScript and Cookies enabled on your browser. You will also need Adobe Acrobat reader, available free from Adobe, to view and print the application form.

Please note if you do not have existing coverage you will need to provide credit card information if you wish to submit your application electronically.


Print an Application

To print out an application for Term Life, Dependent Children Term Life and/or Accidental Death & Dismemberment Insurance, follow these four easy steps:


1.


Determine if your School District participates in the “Premium by Payroll Deduction" administration process by reviewing this School District List. If you would like more information on the premium administration process, refer to the “How will my premiums be collected” section of the Questions and Answers page of this website.


2.


a) If your School District is on this list, open and print Application for Voluntary Term Life, Accidental Death & Dismemberment and Dependent Children Term Life Insurance (PDF - 192 KB) form.

 


b) If your School District is NOT on the list, or if you are a Teacher on Call, open and print the Application for Voluntary Term Life, Accidental Death & Dismemberment and Dependent Children Term Life Insurance – Individually Billed form.


3.


Complete the application in full.


4.


Mail your completed application along with a cheque for your first month's premium, if applicable, to IAP.

To print out an application for Critical Illness or Guaranteed Issue Life Insurance, follow these three easy steps:


1.


Open and print Application for Voluntary Critical Illness Insurance or Application for Guaranteed Issue Life Insurance.


2.


Complete the application in full.


3.


Mail your completed application along with a cheque for your first month's premium, if applicable, to IAP.

Please note you will need Adobe Acrobat Reader, available free from Adobe, to view and print the application forms.
