It is important that you provide banking information so that we can identify:

the bank account in which any funds withdrawn from your High Interest Tax-Free Savings Account may be deposited in the future.

Make the cheque for the initial deposit out TO YOURSELF, like in the sample below. Your cheque must be personalized: your name and address must be pre-printed on it.
